Tay has been going for physiotherapy but the physiotherapist doesn't document anything for the patient, as at now for me I don't know the current improvement on his condition, as the doctors always document when patient are reviewed by them same as the physiotherapist need to document when Tay visit the unit.

Please the hospital management has to type a legal document for discharge against medical advice for patients, witness and doctors to sign before patient can be discharged against medical advice what the patients are bringing outside the hospital is illegal, otherwise in next year the numbers of abscondees will increase.
